Tom Koutsoumpas is a nationally recognized leader in health care policy and advocacy, with deep expertise in Medicare, elder care, advanced illness, and hospice. His career includes influential roles in the U.S. Senate and the Indiana Governor\u2019s Office, as well as leading the Washington presence of major health care organizations. He also the co-founder of Advanced Care Innovations. Throughout his career, he has championed policies that strengthen care delivery and improve quality for patients navigating serious and advanced illness.

Tom serves as Chairman of the Board of Capital Caring Health (CCH), one of the nation\u2019s leading not-for-profit providers of advanced illness, home-based primary, palliative, and hospice care across Washington, D.C., Maryland, and Virginia. He is also CEO of the National Partnership for Healthcare and Hospice Innovation (NPHI), representing more than 90 nonprofit hospice and advanced illness providers committed to patient-centered, mission-driven care.

\nIn addition, Tom co-founded and co-chairs the Coalition to Transform Advanced Care (C-TAC), advancing policies that improve access, equity, and quality of life for people with serious illness. He is also a co-founder and board member of ACT NOW for Mental Health, working to expand access to behavioral health services and reduce stigma in underserved communities. In 2023, Tom was formally recognized for contributions to the space as he was named to Modern Healthcare\u2019s list of \u201cTop 25 Innovators in Healthcare.\u201d

Ryan Beres is an accomplished builder and operator in value-based healthcare delivery and the Chief Executive Officer of Advanced Care Innovations. Ryan has spent the duration of his career in healthcare, mainly focused on operations, strategy, and growth for organizations operating across the spectrum of the managed care including payers, providers, and provider enablement organizations. His prior roles include Director of National Operations at Chenmed, Executive Director of New Ventures at Aledade, and Executive Vice President of Gov\u2019t Programs at CINQCARE.\n

\n\nSince year two of ACIs high needs ACO program, Advanced Illness Partners (AIP), Ryan has been wholly focused on growth and performance of that organization. Under Ryan\u2019s leadership, AIP exploded with over 1100% growth, and nearly $100M in gross shared savings. Dr. Thomas Cornwell is a nationally recognized pioneer in home-based primary care who has made more than 35,000 house calls to over 4,000 homebound patients across a 30+ year career. Dr. Cornwell serves as the Chief Medical Officer of Advanced Care Innovations. In 1997, he founded\nHomeCare Physicians in partnership with Northwestern Central DuPage Hospital, building one of the nation\u2019s leading house call practices.

\n\nIn 2020, he joined VillageMD as National Medical Director for Village Medical at Home, expanding the house call program from one state to twelve and leading its palliative care services. A leader in value-based care, he is known for excellence in documentation, reducing polypharmacy, delivering high-quality end-of-life care, and lowering emergency department visits and hospitalizations.

\n\nIn 2012, he established the Home Centered Care Institute through a transformational philanthropic gift, creating a national education and research organization dedicated to scaling house call programs and developing the workforce. He has mentored hundreds of physicians and advanced practice clinicians.

\n\nDr. Cornwell has received numerous national honors, including the inaugural \u201cHouse Call Doctor of the Year\u201d from the American Academy of Home Care Medicine, the American Medical Association\u2019s \u201cPride in the Profession\u201d award, and the American Geriatrics Society\u2019s 2015 \u201cArnold P. Gold Humanism in Medicine\u201d Award.

\n\nIn 2018, he was recognized in the U.S. House of Representatives\u2019 Congressional Record for his service. Hank Ross is a nationally recognized pioneer in home-based healthcare, focused on expanding access, advancing value-based care, and transforming in-home provider services. He founded Physician Housecalls, leading more than 400,000 house call visits across Oklahoma, Kansas, and Missouri, and also founded and served as CEO of Ross Health Care, one of Oklahoma\u2019s premier hospice and home care organizations.

\n\nWith 14 years of hospital leadership and service in the Oklahoma National Guard, including deployment during Operation Desert Storm, Hank brings broad operational and clinical insight across the post-acute continuum. His work centers on home-based primary care, capitated risk arrangements, and high-needs ACO models serving complex Medicare populations.

\n\nHank currently serves as Executive Advisor for Strategy and Growth at Advance Care Initiatives and as Executive Advisor to the Lincoln Healthcare Intelligence Group. Through Ross Holdings, LLC, he advises leading home care and in-home provider organizations on growth, value-based transformation, and integrating wrap-around primary care services.

\n\nHe plays a key role in supporting house call providers participating in the Academy REACH High-Needs ACO and is helping organizations prepare for CMMI\u2019s LEAD model beginning FY2026.
